Dear
Parents:
Hello and welcome to the start of a new year at
Governor Winslow School. Please feel free to contact me
regarding your child’s health needs at any time. I am concerned
with your child’s complete well-being. Please refer to the
health information section of our Parent Handbook for
additional information.
The following state-mandated screenings of
students will be completed this year. I will begin in October
and will complete the process as soon as possible.

Kindergarten Vision/Hearing Screen

Grades 1-5 Vision/Hearing Screen / Height / Weight

Grade 5 Postural Screening
This is a screening only and is NOT a
diagnostic service. I will notify you by written notice of any
failures or concerns regarding the results. Failures or concerns
regarding results will warrant further assessment by your
child’s pediatrician.
Grade 4:
Your child is in one of the grades in which a
periodic health exam is required by state law. If your child has
had a physical exam within the last year, please send a copy of
the completed physical form to my office. If not, please
schedule an exam with your child’s physician during his/her
fourth grade year. I will be sending out additional information
regarding this at a later time.
Medication Policy:
ALL GRADES-Medication
must be brought to school by parents or an adult, with parental
permission. This is for obvious safety reasons and in accordance
with Marshfield Schools Medication Policy. For
prescription medications, it must
be received in its original pharmacy-labeled container. For
short-term prescription medication, i.e.-antibiotics, written
permission is required only from the parent/guardian as the
original bottle label can serve for short-term prescriptions as
the physician’s authorization. If at all possible, please
schedule this medication dosage before or after school hours.
For all prescription medications that must be
given throughout the school year, the physician and parent
permissions are mandatory. Parental permission is required for
occasional non-prescription medications.
