Welcome to Our Clinic
Daniel Webster School has a beautiful Health Clinic and a full-time
registered nurse, Mrs. Anne Kelly, to meet many of the students' health needs.
Mrs. Kelly is responsible for all health and immunization records. She also
conducts health screenings such as vision and hearing for all grades, and
postural for grade 5.
The Clinic is where your child will go if he/she is ill, injured, or needs
medication. The Marshfield School Committee has a medication policy, which is
strictly enforced. Home and emergency telephone numbers are also kept in the DWS
Clinic. It is important that your child know that someone can be contacted if
he/she needs to be dismissed. This form will also be sent home at the start of
the school year and must be returned as soon as possible.
Medication Policy and Procedure
- School policy does not allow any member of the school personnel to
recommend to administer medication except under exceptional circumstances
wherein the student's health may be jeopardized without it.
- Students requiring medication at school should be identified by parent
and/or the attending physician to the school nurse.
- Students observed by school personnel self-administering unauthorized
medications will be reported to their parents.
- After identification by parent and/or attending physician the following
procedure is mandatory:
(a) Conference with parent/guardian to identify the type, dosage, and purpose
of the medication to be given.
(b) Written statements shall be required of: The attending physician, who will
indicate the necessity of the medication; The parents, who shall request and
authorize school personnel to give said medication in the dosage so prescribed
by the attending physician and thereby release the school personnel from
liability should reaction result from the medication.
- Parents should request from the pharmacist duplicate bottles for
medication if it is to be given during school hours. The bottle kept in school
must contain the name and telephone number of the pharmacy, the student's
name, name of physician, and the dosage of the drug to be taken.
- Taking the medication should be accomplished by the student himself under
the supervision of the designated person.
- Any prescription medication must be brought to the school nurse's office
and signed for by the parent/guardian. At no time may the child bring
prescription medicine to school.
